Psychiatric Rehabilitation Program (PRP)

American Counseling & Treatment Center provides services to assist individuals with mental conditions to reach a higher level of independence. The PRP program assists clients with accessing and coordinating services and resources in their communities and works to ensure that the consumer has access to the best possible treatment. Rehabilitation Staff provide services within the home and the community. Our Staff assist clients with learning and developing improved daily living skills (i.e. personal hygiene, organizational skills, time management skills, nutrition, and money management), as well as working towards achieving healthy coping mechanisms.

PRP for Adults

ACTC provides adult psychiatric rehabilitation services for individuals with severe and persistent mental illness. The program’s goal is to promote independent and community living skills and facilitates the individual’s rehabilitation and recovery. Community Psychiatric Rehabilitation provides flexible service focus areas include:

  1. Development of natural supports through increased opportunities for socialization
  2. Connection to community resources
  3. Navigation of the entitlement system (i.e. SSA, DSS, local housing agencies)/Case management
  4. Community Safety/Advocacy
  5. Life skills training (i.e. grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication)
  6. Development of crisis management and wellness
  7. Assistance with scheduling and maintaining appointments
  8. Coordination of care with other providers/treatment team members (i.e. therapist, psychiatrists, PCP, substance abuse providers, specialists)
  9. Mobility skills training (i.e. assistance with applying for various forms of reduced cost transportation, assistance with understanding the bus system, etc.)
  10. Other areas of need as identified
  • The program serves Adults, ages 18 and up, with severe and persistent mental illness, priority diagnosis.
  • Actively receiving treatment from a physician or licensed clinician for a mental health diagnosis, and who have or are eligible for medical assistance.

Our PRP for Adults offer individuals diagnosed with mental health illness a variety of services to keep them functional in the community while decreasing homelessness promoting stability. The main scope of our adult services is to work with clients in various goals domains outlined below:

Housing: We will work with clients and their families to develop a housing planning and exploration. During the initial session in their home or community. We will assist the client with any housing need. In the case that the client becomes homeless, we will meet with the client in the community and assist with connecting them to services as needed and following up with applications that will need to be completed. Throughout the program, we will work with the client toward a long-term goal of financial security and independent housing in the community of the participant’s choice.

Employment: We will meet with the client to identify vocational interests and goals. We will guide the client throughout employment training by assisting with mock interviews & resume-building, monitoring progress toward vocational goals and processing with the client issues that occurred during the work day. We will assist the client to pursue independent employment by assisting with job applications and follow-up.

Transportation: We will help the client to identify transportation needs and barriers to independent transportation.

Wellness Management: We will help the client to identify substance abuse, mental health and physical health needs. We will work with the client to connect with treatment providers to address these needs and will collaborate with providers throughout the duration of the program to ensure that these needs are being addressed.

Legal Support: We assist the client with identifying a plan to prevent recidivism. Our flied staff will collaborate with outside legal support as needed.

Family and Community Supports: We will help the clients to identify recreational interests, family issues and a plan for stabilization in the community. We will work with the client in culturally appropriate activities and to teach social skills to support family interactions.

Education: We will help the client identify educational interests and goals. ACTC will guide the client throughout educational training and will help the client to independently seek additional educational resources.

Entitlements, financial assistance and budgeting: We will help the client to identify financial needs. We will work with the client to access entitlements and financial assistance and to follow up with applications and recertification date’s. Our field staff will be trained to help the client move towards financial security by developing a budgeting plan.

Therapy compliance: We will work with the client to make sure they actively participating in therapy and maintaining contact with psychiatrist services as needed. ACTC therapy compliance specialist will assist the client with staying connected with mental health counseling.

Illness Management: ACTC offers a holistic approach to serving clients; we will assist the client with keeping up with all medical appointment is order to address all medical needs.

Social Skills Development Include:

  • Community integration activities
  • Developing natural supporting & activities to support individual’s cultural interests
  • Group and self-directed leisure activities related to sports and arts
  • Activities related to client’s spiritual interests

ADULT SERVICES

  • Development of community living skills.
  • Coordinating delivery of mental health services.
  • Medication education and monitoring.
  • Development of age appropriate self-care skills.
  • Assistance with entitlement and resource education services.
  • Vocational services.
  • Social Skills development, including community integration services.
  • Health Promotion & Training.
  • Promotion of individual wellness self-management and recovery.

Our Services are designed to:

  • Maximize independent functioning.
  • Promote the recovery and self-determination process.
  • Increase interagency coordination and collaboration in all aspects of the treatment planning process.
  • Reduce inpatient hospitalizations and out of home placements.

Therapeutic Behavioral Services

ACTC’s Therapeutic Behavioral Services (TBS) Program is an intensive, individualized, service for children up to the age of 21, who are diagnosed with a mental illness or developmental disability. The program focuses on decreasing maladaptive behaviors such as ; behaviors that are harmful to oneself or others, developmentally inappropriate, and disruptive or dangerous. TBS is provided in the family’s home and in the community.

Therapeutic Behavioral Services include up to 40 hours a week of In-Home and Community services for the child and their family.

Autism and Related Services

ACTC’s Autism and Related Services are designed to serve school age children up to 21 years of age, who have been diagnosed with Autism Spectrum Disorder. The program services work directly with the child and their family to support the development of skills to live, play and socialize with increased independence at home and in their communities.
